Brief summary of purpose:

The Clinical Supervisor is responsible for oversight of daily operations related to the medical clinical and ADHC.

Oversees implementation of discipline specific policies and procedures.

Supervises and directs Medical Assistants and Health Aides

Possesses nursing assessment skills, ability to work within an interdisciplinary team model and knowledge of the nursing care planning process. Excellent interpersonal communication and problem-solving skills. IV skills recommended but not essential. Functions independently in the day-to-day management of the clinical needs of participants and the operations of the PACE center clinic utilizing Fallon Health and Summit ElderCare policies and procedures, leadership guidance and program goals in the decision-making process.

Responsibilities

Primary Job Responsibilities:

  • Responsible for recruitment and training of staff within the discipline including Medical Assistants and Health Aides.

  • Participates in the oversight of related contracted vendors.

  • Represents the program and related discipline at assigned community and external events upon request.

  • Evaluates staff on achievement of goals and deliverables and assessment of competencies. Manages the resolution of performance issues in consultation with the Clinical Manager and Human Resources as appropriate.

  • Provides ongoing constructive feedback and guidance to staff.

  • Conducts new staff orientation and performance evaluations for staff in a timely manner.

  • Coordinates the provision of patient care and scheduling within the medical clinic setting including PACE providers and ancillary providers (dentistry, podiatry, optometry, audiology).

  • Collaborates with participants, caregivers, physicians, nurse practitioners and other staff to meet participants’ needs.

  • Oversee Health Aid scheduling for functional maintenance programs.

  • Oversee scheduling of INR’s and community blood draws for Medical Assistants

  • Participates in all types of Interdisciplinary Team meetings.

  • Participates in management and quality meetings as required.

  • Coordinates/supervises the purchase of medical/surgical/pharmaceutical/food service supplies.

  • Assures that clinical equipment is in good working condition in collaboration with the Quality and Risk Manager.

  • Assumes responsibility for inservice education for medical assistants and health aides.     

  • Interviews potential candidates and makes recommendations for hiring to Clinical Manager. Provides for cultural and diverse needs of participants when providing care.

  • Monitors and evaluates therapeutic interventions and identifies emergency situations and initiates appropriate nursing orders/interventions

  • Participates in the nursing “on-call” rotation as assigned.

  • Supports RN colleagues, covering and assuming changes in assignment as assigned by Clinical Manger.

  • Conducts regular staff meetings with direct reports.

  • Assists with routine nursing duties as required.

  • Assures that corrective action plans are completed for incidents related to clinical services.

  • Assists the Clinical Manager to carry out administrative duties as needed. Represents the Clinical Manager at meetings as assigned. Acts on behalf of the Clinical Manager in his/her absence.

  • Performs all duties in accordance with FCHP and Summit ElderCare policies and procedures. Strictly observes HIPAA regulations and confidentiality of participant information.

  • Oversees and ensures the quality checks by the Medical Assistants and Health Aides are completed as required and in accordance with SE policies and procedures.

  • Participates in site specific meetings per the direction of the Clinical Manager or Site Director

  • Completes MDS assessments as indicated.

  • Performs nursing assessments as needed at the center or in the community.

  • Completes level one and level two reports, when necessary to assist & offload the Clinical Manager

  • Performs other responsibilities as assigned by the Clinical Manager/Site Director.

  • Participates in RN On Call Rotation

Qualifications

Education:

Graduate of an accredited school of nursing

License/Certifications:

Licensed as an RN in Massachusetts

Certification: CPR and Alzheimer and Dementia certification preferred or willingness to be certified

Experience:

At least two years of supervisory experience working with frail elders in a health care setting. * *

Supervisory experience with medical assistants and health aides is preferred.

Ambulatory clinic experience is recommended but not essential
