Job Summary:
Provide skilled occupational therapy to patients in their homes, improving independence in daily activities. Develop and implement treatment plans, assess progress, and educate patients and caregivers.
Key Responsibilities:
Evaluate patient needs and create customized therapy plans.
Provide hands-on therapy to restore function and mobility.
Educate patients and caregivers on exercises and adaptations.
Document patient progress and collaborate with the care team.
Ensure compliance with healthcare regulations and best practices.
Qualifications:
Licensed Occupational Therapist (OTR/L)
Experience in home health preferred
Strong communication and assessment skills
Minimum Qualifications:
Graduate of an accredited occupational therapy program.
Eligible for NBCOT exam; registered if required by state.
Ability to work independently and maintain professional relationships.
Commitment to ongoing skill development and policy adherence.
At least 1 year of experience or 6-8 weeks with a preceptor.
Home healthcare experience preferred.
Valid driver’s license and reliable transportation.
