Description

Under direction, the Community Services Coordinator actively engages with communities to provide education and support in areas such as social work, community health, mental health, and education. The incumbent’s responsibilities include developing and implementing programs, conducting training sessions and workshops, and fostering community outreach efforts.

EDUCATIONAL/BILINGUAL INCENTIVE: Possible educational incentive of 2.5%, 3.5%, or 5% based on completion of Associate's, Bachelor's, or Master's degree. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only; there is currently one (1) Full-Time Regular vacancy.

Examples Of Duties

Duties may include but are not limited to the following:

  • Develops, administers, supervises, and evaluates community education, outreach, and engagement activities;

  • Plans, organizes, and coordinates programs, staff activities, and community outreach activities;

  • Conduct or evaluate needs assessments, initial screenings and health histories of clients/participants;

  • Administer fingerstick procedures to obtain adequate blood samples for glucose testing;

  • Serves as liaison with representatives from state, County, and local agencies, community-based partners, and stakeholders to identify needs and resolve problems;

  • Interprets and implements federal, state, and county laws, regulations and policies to ensure adherence to state and County program and fiscal guidelines;

  • Prepares a variety of reports and correspondence related to the program; and

  • Performs other related duties as required.

Typical Qualifications

These are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list.

EDUCATION, TRAINING, and EXPERIENCE:

Considerable experience in social work, community health, mental health or education which has led to the acquisition of the required knowledge and abilities.

The required knowledge and abilities also can be obtained by:

  • Four (4) years of experience in social work, community health, mental health or education, including responsibility for program development and implementation.

  • At least one (1) year of the qualifying experience must be in community outreach and education.

Necessary Special Requirements

  • Must possess experience conducting classroom training and educational workshops

  • Must be bilingual (English/Spanish)

  • Must possess and maintain a valid California Driver’s License

  • Considerable skills in Microsoft Word, Excel, and PowerPoint

  • Must possess excellent written and oral communication skills

  • Must possess excellent public speaking skills

  • Must possess community engagement work experience

  • Must possess a phlebotomy license

  • Must be a certified Lifestyle Coach for Diabetes Prevention Program

    Desired:

  • Bachelor's degree in Public Health or a related field.

Knowledge, Skills, and Abilities:

Thorough knowledge of: community organization and resources; diagnostic, counseling, and referral techniques; group dynamics; principles of supervision and case management; safe driving practices; federal and state laws and ordinances applicable to the Diabetes Prevention Program area.

Skill in: performing fingerstick glucose monitoring, employing accurate and reliable techniques to obtain blood samples for glucose level assessment; Microsoft Office products including Word, Excel, and PowerPoint.

Working ability to: develop educational programs and evaluation plans, specifically in diabetes prevention; coordinate activities among various groups; plan and organize work of others; conduct classroom training or educational workshops; communicate effectively orally and in writing, communicate verbally in both English and Spanish; and exercise sound judgment.

TRAINING & EXPERIENCE EVALUATION – 100%

A Training and Experience Evaluation (T&E) is a structured evaluation of the job application materials submitted by a candidate, including the written responses to supplemental questions. The T&E is a method for determining the better qualified applicants among those shown to meet the stated requirements. Using a T&E, applicants may be scored or ranked according to criteria that most closely meet the business needs of the department. When the pool of candidates is exceptionally strong and large, candidates are typically scored or ranked in relation to one another; consequently, some qualified candidates may receive a score or rank which is moderate or lower resulting in them not being advanced in the process.

NOTE: In a typical T&E, your training and experience are evaluated in relation to the background, experience and factors identified for successful job performance during a job analysis. For this reason, it is recommended that your application materials clearly show your relevant background and specialized knowledge, skills, and abilities. It is also highly recommended that the supplemental questions within the application are completed with care and diligence providing all information requested. Examinees must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.

If there are three (3) or fewer qualified applicants, a T&E will not be conducted. Instead, a score of seventy percent (70%) will be assigned to each application, and each applicant will be placed on the eligible list.

Applicants successfully completing the exam process may be placed on an eligible list for a period of one (1) year.
