Position Summary Information
Job Description Summary
The Community Health Worker helps the students, their families; teachers, administrative staff and stakeholders gain knowledge of the School Based Health Center Program services and resources.
Minimum Qualifications
Associates Degree in Human Service. Healthcare education and/or training. Relevant experience can be substituted for formal education. Outreach and/or enrollment experience required. Must have access to reliable transportation. All staff working within the School Base Program must be fingerprinted and receive successful clearance through the New York State Department of Education who will complete criminal background checks.

Preferred Qualifications
Bilingual preferred.
Physical Demands
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
