UnitedHealth Group Nurse Practitioner - Community Center Clinics and Embedded Nurse Practitioner Program in Surprise, Arizona
The Nurse Practitioner (NP) for our Community Center Clinics and Embedded Nurse Practitioner Program will be primarily completing Medicare Annual Wellness Examinations as part of the Optum Medicare Advantage Insurance Network. The NP must demonstrate successful clinical practice in assessing patients in the senior age groups and those with permanent disabilities. Also, the NP has the capability to diagnose, confirm and/or reject prior diagnosis, develop treatment plans, and evaluate current treatment plans. This NP must have documentation which achieves completeness, technical accuracy, and specificity resulting in accurate Hierarchical Condition Category (HCC) coding. This NP must be able to work completely independent. There will also be times this NP will be required to work with a Medical Assistant. This NP will also develop and present at least one educational topic annually in partnership with our Community Center Educational Programs.
Primary Responsibilities :
Completing Medicare Annual Wellness Exams including full patient assessments on our Medicare Advantage senior/disabled patient population
Connect with and educate the senior/disabled population in a community center clinic, physician office, or virtually (can be done from your home)
Travel is required to our different community center clinics, physician offices, and rarely to patient’s homes
Completing and/or ordering recommended screening test (i.e., QuantaFlo, HgBA1c, Monofilament, Retina-Vue and Bone Density, etc.), interpreting the results, communicating the results to the patients and if needed to the patient’s Primary Care Provider (PCP)
Ability to formulate a differential diagnosis and assign ICD-10 diagnosis code to highest level of specificity
Proficiency in assessment and development of treatment plans and associated clinical documentation requirements
Identifying gaps in care or other unmet needs and making the appropriate referrals or assisting in closing those gaps
Complete on average 8 – 9 patient visits a day
Required Qualifications:
Arizona Nurse Practitioner license and national certification (Family, Geriatric, or Adult Medicine Certified Nurse Practitioner)
Proficiency in assessment and development of treatment plans
Arizona Driver’s License and Auto Insurance in accordance with the state of Arizona (or will be licensed and insured in Arizona by employed date)
Ability to lift a 30-pound bag in and out of a car to navigate stairs and a variety of clinical spaces.
Ability to work autonomously
Ability to work in a team environment
Basic computer proficiency
Familiarity with electronic medical record systems
Familiarity with HEDIS/Star Measure guidelines
Familiarity with coding and documentation
Familiarity with health conditions frequently found in 65+ aged population
Preferred Qualifications:
Previous experience completing Medicare Annual Wellness Exams
Knowledge and or experience working with the Medicare Advantage Population performing patient assessments
5+ years of clinical experience as a Nurse practitioner
Experience in Gerontology, or Cardiology, or Internal Medicine, or Endocrinology
Home Care or home visit experience
