Description

The Division of Cardiothoracic Surgery is seeking an Advanced Nurse Practitioner/Physician Assistant for our lung cancer screening program with Stony Brook Cardiothoracic Surgery. The APP will conduct a comprehensive exam and direct patients to the most appropriate treatment options in collaboration with attending physician(s). The APP will work collaboratively in a multidisciplinary environment, functioning as a liaison between Thoracic surgery, Pulmonary, Radiation Oncology, Internal Medicine and other physician practices and hospital departments.

Job Duties & Responsibilities

  • Independently conduct in person or telehealth visits with patients, acting as a patient liaison to guide patients and families through the screening, enrollment and treatment process.
  • Communicate with referring physicians regarding patient appropriateness for program and patient's progress once enrolled.
  • Order CT scans, coordinate with radiology services and contact patients with results and treatment plans.
  • Understand, facilitate and communicate effectively with the different entities participating in each patient's care plan.
  • Participate in multidisciplinary discussions to review overall program statistics and discuss individual cases, including attendance at monthly program meetings and weekly case conferences.
  • Maintain a level of knowledge to discuss benefits and harms of CT lung screening and lung nodule management; keep abreast of current literature.
  • Analyze clinical practice patterns to provide the best medical care and to increase effectiveness and efficiency.
  • Communicate verbally and in writing all pertinent patient information to supervising physician, nursing staff, patient resource manager, home health agency, and other relevant healthcare providers.
  • Provide timely documentation in the electronic medical record of all relevant data including results of diagnostic tests; laboratory results; patients' condition and response to therapies/interventions; communications with physician colleagues.
Qualifications

Required Qualifications:

Nurse Practitioner: Completion of an accredited Nurse Practitioner program, with a master's degree (foreign equivalent or higher) in nursing. Must have current or compact RN licensure in the state of New York. National certification as a NP required prior to start date.

Physician Assistant: Completion of an accredited Physician Assistant program. Master's degree (foreign equivalent or higher) required. Must be licensed in the state of NY, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.

Preferred Qualifications:
Preference for candidates with doctorate degrees (or foreign equivalent) and 5 years of clinical experience. Excellent communication skills.
