Description:
Assist in planning, organizing, implementing, evaluating and directing overall operation of the Social Service Department according to federal, state and local standards, guidelines and regulations, established policies and procedures, etc. to assure the medically related emotional and social needs of the residents are met/maintained on an individual basis.

Key Responsibilities:
Inspire and Lead: Demonstrate Sanctuary Values by setting an example for internal and external customers, creating a calm and supportive environment throughout the community.
Collaborative Culture: Execute assignments in a collaborative and shared culture across all divisions within TSLC.
Empathetic Engagement: Conduct private interviews with residents/families to understand social history, actively participate in community planning, and address concerns through courageous conversations.
Holistic Care: Play a crucial role in discharge planning, developing and implementing social care plans, and conducting cognitive evaluations to ensure comprehensive resident care.
Innovative Solutions: Assist in developing, delivering, and following up on written plans of care, identifying problems/needs and establishing achievable goals for residents.
Continuous Improvement: Participate in community surveys, develop corrective plans for department deficiencies, and contribute to ongoing enhancements in our service delivery.
Minimum Qualifications:
Bachelor's or master's degree in social work or a related human services field.
One year of supervised social work experience in a healthcare setting.
Previous experience working with the geriatric population is preferred.
Superior customer service skills and professionalism.
Desired Skills:
Flexibility to thrive in a fast-paced, customer-driven environment.
Excellent written and verbal communication skills.
Proficiency in Microsoft Office Suite.
Ability to make independent decisions and work effectively with residents and personnel.
Physical and Mental Requirements:
Adaptability to changing work parameters.
Ability to see, hear, and smell, or use prosthetics to enable these senses adequately.
Physical agility, dexterity, and coordination.
Ability to lift a minimum of 25 pounds and assist in emergency situations.
Working Conditions:
Exposure to noise, infectious waste, diseases, etc.
Ability to speak, read, and write English fluently.
Capacity to cope with mental and emotional stress.
