Urban League of Metropolitan Seattle Case Manager - Health Through Housing in Seattle, Washington

Job description

Safe, habitable, and affordable housing is a basic need. The Urban League of Metropolitan Seattle (ULMS) Housing Department provides direct service programs and resources to individuals and families through shelters, transitional housing, rental assistance, and homeownership programs.

ULMS is actively hiring a Case Manager for our Permanent Supportive Housing Site in the West Seattle/Westwood Village area. The Case Manager supports HTH residents in accessing support services, community integration, life skill training, housing maintenance, transportation, and other needed services. This role provides day-to-day case management for residents who are transitioning from homelessness to safe, secure, long-term housing. The Case Manager is primarily responsible for assisting residents with their daily and ongoing needs, particularly with challenges that arise which may be related to their severe and persistent mental illnesses and/or substance use disorders. Using Housing First, Harm Reduction and Advanced Motivational Interviewing models of case management that is client driven, solution focused and facilitates change and movement toward greater stability and well-being, the Case Manager will work with the residents to develop a list of key challenges, needs, and interests and then establish specific and measurable goals around them. The Case Manager coordinates the continuity of services for each resident on their caseload. This includes coordinating care planning, completing a comprehensive assessment of needs, developing a Case Management Plan, monitoring the effectiveness of it, and ensuring resource linkage, referral, and advocacy.

Schedule is generally Monday - Friday 8:00AM-4:30PM, however adjustments may be made to accommodate case management needs

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  • 2+ years of experience in social services case management
  • High School Diploma or GED; Bachelor's degree in Social Work, Psychology or related human service field preferred
  •  Knowledge of HIPAA compliance when collecting and storing participants information.
  • Knowledge and ability to access entitlement programs, community, social, medical, mental, chemical dependency services and/or education, job training and employment services within ULMS and outside the organization as appropriate based on client(s) need. 
  • Knowledge and ability to successfully implement interviewing, counselling methods, principles, and techniques that result in measurable positive outcomes.
  • Ability to analyze interventions for effectiveness, research and develop alternative interventions, openly solicit supervisory guidance and input.
  • Proven ability to maintain accurate, thorough, timely and complete case files and ability to communicate professionally in writing and verbally; Knowledge of electronic recordkeeping for client information.
  • Demonstrated ability to work with difference, specifically around race, ethnicity, religion, sexual orientation and gender identity, disability, etc., and how these differences contribute to disparate outcomes for these populations.

ULMS Total Compensation

Most Benefits will take effect on the 1st of the month following a 60-day waiting period** 

 Medical - $8,741.40 per year 

Dental - $570 per y
