This Day Habilitation - Case Manager position is located in Schenectady County.

The Day Habilitation Case Manager is responsible for the development and review for all Day Habilitation Plans and “Outcomes” at ourSchenectady Site Base Day Habilitation Program.

The Day Habilitation Coordinator will also ensure “Outcomes” are being carried out according to the Day Habilitation Plan and will act as an advocate for all individuals enrolled.

Job Responsibilities:

  • Develop and write Day Habilitation plan for each Participant.

  • Rewrite the Day Habilitation plan to reflect changes.

  • Coordinate and review the Participant’s plan with the family, Service Coordinator, and Participant on a semi-annual basis to discuss progress/regression and overall performance within Day Habilitation.

  • Communicates with the Participant and/or provider to ensure that the Day Habilitation plan of service meets the Participant’s needs and desires; supplies guidance or direct intervention when it does not.

Requirements

  • Bachelor’s Degree in human services and one (1) year of experience in providing services to persons with developmental disabilities.

  • Good writing, typing and interpersonal communication skills.

  • Valid NYS driver’s license and reliable transportation.
