Type: Hybrid (Fieldwork required up to 50%)
Compensation: $20.67 - $23.08 per hour

What You'll Do:
As a Case Manager, you'll be at the forefront of our Population Health Program, working with members in the community to improve their physical and behavioral health outcomes. Your role is vital in helping individuals achieve their health goals through personalized care, tailored interventions, and a deep understanding of their medical, behavioral, and social needs. By partnering with health plans, you'll also ensure clinical and financial outcomes for the broader population.
Empower Members: Support individuals by developing care plans that reflect their unique health and social needs. Help them navigate the healthcare system and encourage self-management of their health.
Make a Difference: Address the specific needs of people with SMI and SUD, working closely with them to reach personal health goals, improve wellbeing, and enhance their overall quality of life.
Work with a Team: Collaborate with health plan partners and the Mindoula team to deliver integrated care that improves outcomes for the community.
Fieldwork Focused: Spend up to 50% of your time in the field, directly engaging with members and delivering personalized case management services in the community.
Ideal Candidate:
We are looking for someone with an entrepreneurial mindset and a passion for supporting people. If you have experience working in case management or related fields and enjoy a challenge, we want to hear from you! You'll excel in this role if you're self-motivated, detail-oriented, and able to work independently while juggling multiple priorities.
Education & Experience Requirements:
Bachelor's Degree in a relevant field (e.g., Social Work, Psychology, Nursing, etc.).
At least 2 years of experience in case management, behavioral health, or a related field.
Strong understanding of Social Determinants of Health, Mental Health, Substance Abuse Disorders, and Violence issues.
Excellent communication skills—both verbal and written.
Comfortable working with Google Suite (Sheets, Docs) and Microsoft Office (Excel).
High attention to detail and ability to multitask under pressure.
