Case Worker - Glen at Hiland Meadows

Part Time – 20 Hours- Days (some flexibility)

What you will do:

The caseworker works with the social worker to ensure that residents and families of the Glen at Hiland Meadows needs are met. Will participate as a member of the multi-disciplinary team to support residents and their families by providing support as well as resources available in the community. Act as a liaison between the residents and families and the departments at the Glen at Hiland Meadows. Will assess circumstances and needs of the residents on an ongoing basis and communicate with the Social Worker, the RN Manager, RN Director, and Executive Director as needed.

Responsibilities:

  • Assists social worker/licensed staff with the assessment of resident needs, connecting residents and their families with resources available at the Glen at Hiland Meadows as well as in the community.

  • Works as a liaison between residents and their families and the Glen at Hiland Meadows departments.

  • Act as a resident advocate within the interdisciplinary team.

  • Regularly attends case management and State of the IL resident meetings as needed.

  • Communicates with the Social Worker, the RN Manager, RN Director and Executive Director as needed.

  • Maintains a positive working relationship with all departmental staff within The Glen at Hiland Meadows.

  • Participates constructively in interdisciplinary meetings and committees and other resident centered care teams as necessary.

  • Assists with arranging and conducting resident/family meetings.

  • Serves as resource to focused resident care teams regarding resources available in resident’s community .

  • Serves as resource to patient focused care teams regarding issues and regulations related to end of life care/advance directives.

What you will need:

  • Bachelor’s degree in Social Work, sociology, psychology, LPN licensure or related field required.

  • Minimum of 1 year of experience is required

Pay Range: $23.40 - $33.69

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
