HCR Home Care CNA - Begin a Caring Career with HCR - Clinton County in Plattsburgh, New York

HCR offers a 1 day - August 16th - PAID certification class for CNAs to obtain their Home Health Aide Credentials. Starting rate is $18.55/hr. plus additional incentive pay available

Summary: Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.

Essential Responsibilities:

· Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.

· Immediately report changes in client’s condition to supervising professional.

· Complete and submit necessary documentation including charting tasks and observations on android device.

· Attend ongoing in-service programs in a timely manner to maintain certification.

· Ensure the safety of the patient.

· Maintain HIPPA.

· All other duties as assigned.

1 Day Transition Class Requirement:

· Valid NYS CNA certificate required to participate in the 1 day CNA to HHA transition class.

· Must be mature and able to deal effectively with the job and its demands. Must be respectful of the patient’s rights and treat a client and his/her belongings with dignity and respect. Have empathy and compassion for the ill and elderly.

· Valid Driver's License and own reliable vehicle to make patient visits.

· B e available for work and accept assignments when offered.

· Be able to speak and write in English in order to follow the plan of care, communicate effectively with patient and interdisciplinary team.

Physical Requirements:

The physical demands described here must be met by an employee to successfully perform the essential functions of this job, and to ensure patient safety at all times in the household. While performing the duties of this job, the employee is required to visually recognize objects within 20 feet, have the ability to speak and hear.

· Frequent walking and standing (80%-100% of the time)

· Frequent lifting

· Must be able to lift / move 40 lbs. unassisted

· May require pushing / pulling / transferring 10 lbs. of weight

· Sufficient hand strength and dexterity to carry out care plan duties

· Ability to bend, stoop, reach, pull and push unsupported, routinely.

· Ability to climb two flights of stairs

· Working conditions may expose the home health aide and / or personal care aide to smoke, pets and other allergens.

· Potentially hazardous materials such as those found in cleaning agents and other chemicals may be stored in a client’s home.
