Crossroads Hospice & Palliative Care Hospice Chaplain in Parma, Ohio

Hospice Chaplains play a vital role in guiding patients and families through the sacred and transformative journey of end-of-life care. As part of our dedicated team, chaplains provide a compassionate and calming presence, whether through brief moments of connection or extended time spent offering support. Their work helps ensure patients feel spiritually comforted and at peace as they approach life’s final chapter.

By listening deeply, embracing silence when needed, and engaging in meaningful conversations, chaplains assist patients in exploring life’s essential questions, finding significance in their experiences, and discovering a sense of comfort. They work closely with an interdisciplinary care team to address not only physical and emotional needs but also spiritual concerns, honoring all beliefs with respect and without intent to convert.

Our commitment extends beyond the patient, providing ongoing support to families for up to a year after their loss. Hospice Chaplain Requirements:

  • Ecclesiastical endorsement by faith group/denomination

  • Minimum of two units of Clinical Pastoral Education (CPE) from an Accredited ACPE Center

  • Master of Divinity degree or equivalent preferred

  • One year of hospice care experience preferred

  • Experience providing spiritual care to terminal patients, families, and clinicians

  • Familiarity with hospice philosophy of care and end-of-life practices across faiths

  • Excellent verbal and written communication skills

  • Ability to work independently and collaboratively as part of a hospice care team

  • Proficient in electronic medical documentation for care plans and interventions

  • Valid driver’s license and access to reliable, insured transportation

Hospice Chaplain Responsibilities:

  • Provide or coordinate spiritual care for patients, families, and caregivers.

  • Conduct spiritual assessments in a timely manner.

  • Support bereavement follow-up in collaboration with the bereavement coordinator.

  • Actively participate in care conferences, hospice admissions, and IDG meetings.

  • Offer emotional/spiritual support to hospice team members as needed.

  • Maintain relationships with local spiritual leaders.

  • Contribute to quality improvement programs.

Hospice Chaplain Schedule & Benefits:  

  • Schedule: Monday to Friday, 8:30AM - 5:00PM (on-call rotation that will include weekends and evenings).
