The community you’ll be caring for: AdventHealth Palm Coast Parkway 1 AdventHealth Way, Palm Coast, FL 32137

The role you’ll contribute: The Care Management Supervisor is under the general leadership of the Care Management Manager or Facility Director and assists in monitoring and performing the daily operations and supervision of team members in the Care Management department.

The value you’ll bring to the team:

· Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting as directed by Care Management Manager or Facility Director including direct supervision of Care Management staff

· Assists Care Management Manager/Facility Director to implement the Corporate Care Management strategies

· Ensures active staff participation in daily Multidisciplinary Rounds

· Serves as a point of contact for all identified stakeholders

· Serves as a content resource regarding medical necessity criteria, patient status and discharge criteria

The expertise and experiences you’ll need to succeed :

Minimum qualifications :

· Associate's Degree Nursing or Bachelor's Degree Social Worker

· Registered Nurse Florida Licensure and/or Compact State Licensure

· At least 2 years of experience in direct Care Management

Preferred qualifications:

· Bachelor's Degree Nursing

· Master's Degree Social Worker

· At least 3 years of experience in direct Care Management

· RN-BC in Care Management

· CCM Certified Case Manager

· ACM Certified Case Management
