Liberty Healthcare Corporation MSW Consultant - assessment & service coordination (hybrid) in PA, United States

Liberty is hiring professionals with Master of Social Work degrees for several new Quality Reviewer positions which will provide technical assistance to Service Coordinators who conduct assessments using the interRAI-HC tool.

This is a hybrid position which will require you to work in the field and travel to various locations throughout the region approximately 50% and allow you to work from your home office approximately 50%.

The work schedule for this position is Monday through Friday during daytime hours.

Responsibilities include:

  • In-person and virtual training and technical assistance with Service Coordinators

  • Applying subject matter expertise with the interRAI-HC tool

  • Ensuring best practices are used in the administration of the interRAI-HC tool

  • Conducting quality reviews of assessments and service plans

  • Proficiency testing

  • Developing and implementing performance improvement plans and monitoring efficacy

  • Collaborating with supervisors, managers, trainers, and educators

  • Data analysis and reporting

    As a full-time employee of Liberty Healthcare in this role, you can expect:

  • Annual salary of $90,000

You are a good fit for this role if you have a Master of Social Work degree and have experience in these areas:

  • Conducting functional assessments of older adults and adults who are living with disabilities

  • Home and Community Based Services (HCBS) or Long Term Services and Supports (LTSS)

  • Case management and service coordination

  • Leadership, management, supervision, or consulting

  • Technical assistance and training

Subject matter expertise with the interRAI-HC tool is strongly preferred and highly valued.
