Job Description
Praxis Healthis seeking a dedicated and compassionate Registered Nurse to serve as aRemote Care Management Supervisor. In this role, you will oversee an excellent group of care coordinators and manage population-based care management programs.
Location:Remote position for an RN located in thestate of OR or SW Washington
Pay:$42-46 per hour DOE
Schedule:Monday-Friday, 8am-5pm
This is a working supervisor role with approximately 20-30% FTE spent on direct patient engagement and documentation. The remaining 70-80% FTE will be focused on managerial tasks such as scheduling, performance reviews, volume tracking, and timecards as well as project management.
This position interacts with both internal and external stakeholders which requires prompt and professional communication. Monitoring these programs requires Microsoft Excel skills, problem solving, and creativity. We are looking for a nurse who has previous management experience who is excited about the future of population health nursing!
This role collaborates closely with, and reports to the Director of Population Health. Collaboration includes review of care management performance and development of new care management interventions to improve patient outcomes and reduce medical spending.

Essential Functions, Roles and Responsibilities
Manage a team of care coordinators including monitoring the quality of their work and providing constructive feedback
Review and analyze data to monitor the effectiveness of our care management programs and ensure they are meeting expected goals and outcomes
Communicate care management goals, outcomes and processes to the clinics and care teams
Collaborate with the Director of Population Health to identify high risk patient population in need of care management services
Assist with designing and implementing care management processes to engage high risk patients, reduce utilization and improve health outcomes
Directly engage with patients to assist with care management goals when needed
Document in the EMR
Participate in monthly meetings with various insurance companies to discuss high risk patient populations
Collaborate with clinical care teams including providers, medical assistants, social workers and others
Collaborate with the operations and management teams as needed to manage care management projects
Assist with other project and tasks as needed

Minimum Qualifications
Bachelor’s degree in nursing
Current, unrestricted Oregon RN license and willing to get licensed in Washington and Idaho upon hire
Care Management certification preferred
Minimum five years of nursing experience; outpatient and inpatient nursing experience preferred
Previous management experience
Previous use of Greenway Intergy EHR preferred; Must be able to navigate multiple EHR systems
Microsoft Excel and PowerPoint skills

General Physical Requirements
The Remote Care Management Supervisor role requires the ability to work primarily in a seated position, utilizing a computer for extended periods. Candidates must have good visual acuity for reading and interpreting documents, dexterity for operating office equipment, and effective communication skills for interactions via phone, video, and written correspondence. The role demands strong time management skills to handle multiple priorities independently, and the ability to promptly respond to urgent communications if necessary.
