Title: Social Worker, Hospice (4)Position Type: Permanent, Full-timeAgency: New York City, Non-Profit Health SystemLocation: Field Based - Bronx, Queens, and ManhattanHours: (TBD)Salary: $68K-82K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of three (3) yearsOur client, a non-profit healthcare services provider, is in need of four (4) Social Workers. They are to provide psycho-social support counseling and other social work-related services to hospice and palliative care patients and their families in the community homecare setting. Refer to hospice whenever appropriate.Requirements:Social Work license (LCSW), Palliative Care Certificate, Pediatric SpecialtyThree (3) years of experience in Social Work home care or hospice preferredSocial Work Degree & LicenseBilingual skills (Spanish, Cantonese or Mandarin)Customer Service: Strives to provide the best customer service at all times to internal and external customersCommunication: Utilizes appropriate communication methods and style to ensure information is efficiently and effectively delivered and receivedCollaboration/Cooperation: Works well with others and takes actions that are most appropriate for achieving departmental and organizational goalsDependability/Accountability: Meets productivity, attendance, and punctuality standardsProfessionalism: Demonstrates MJHS core values, adheres to the MJHS Code of Conduct and Standards of ConductCOVID vaccination is required
