Amergis Public Health Nursing Consultant in Nashville, Tennessee

The Public/Community Health Nurse (PHN) is responsible for the mental and physical well-being of the community by ensuring the public’s health through health promotion, disease prevention, treatment, and education. They can work in a variety of institutions such as the community, hospitals, clinics, or long-term care facilities while collaborating with state regulators, doctors, and other healthcare professionals to keep the community safe and healthy.

Essential Duties and Responsibilities:

  • Assesses health care trends in communities

  • Collaborates with emergency response planning and training when applicable

  • Works with public health officials to help communities access and receive care

  • Provides immunizations and health screenings to all populations in the community

  • Provides patient and community education

  • Monitors and delivers high-quality patient care

  • Records and analyzes medical data in various communities

  • Evaluates the health of patients and develops treatment plans in collaboration with medical providers and multidisciplinary team members

  • Participates in the evaluation and documentation of patients and care

  • Assists in developing, evaluating, and implementing professional development education

  • Refers patients to other providers as needed

  • Manages budgets of public health facilities if applicable

  • Reviews the work of assigned team members for thoroughness, soundness of recommendations, and compliance with the standards, policies, procedures, and regulations of the CDC as well as the County Department of Public Health

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • Active Nursing Licensure in the state practicing

  • Two (2) years of public health or community experience preferred

  • Complies with all relevant professional standards of practice

  • Participation and completion of Amergis' Competency program when applicable

  • Current CPR if applicable

  • TB Questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or state regulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable to job site

  • Understand patient confidentiality and HIPAA requirements

  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

  • Computer proficiency required

  • Must be at least 18 years of age
