Job Title:

• Primary Home Care (PHC) Field Supervisor

Job Type:

• Full-Time

Key Responsibilities:

• Conduct routine on-site home visits to ensure the quality of care and compliance with company standards.
• Provide training, mentorship, and ongoing support to caregivers to maintain a high standard of service.
• Assess client needs, create care plans, and ensure the implementation of individualized services.
• Serve as a liaison between clients, caregivers, and the administrative team to address concerns and improve services.
• Monitor caregiver performance and provide timely feedback for professional growth.
• Maintain accurate records of visits, evaluations, and incidents for compliance and reporting purposes.
• Coordinate schedules and resolve staffing challenges to ensure uninterrupted client care.
• Assist with client intake assessments and ensure all necessary documentation is completed.
• Promote a safe and healthy work environment for clients and caregivers.

Requirements:

• Minimum 6 months of previous work experience in related field.
• Valid drivers license (Required).
• Ability to commute/relocate to Palmview, Texas, reliably or with plans to relocate before starting work (Required).
• Willingness to travel across the Rio Grande Valley (Company car provided).
• Minimum High School Diploma.
• Nursing license (Preferred but not required).
• Bilingual in English and Spanish (Required).

• Experience in home care or healthcare management is a plus.
