Community Resource Navigator Description

Position: Community Resource Navigator
Location: Marion, Ohio
Supervisor: Michelle Ryman-Lillie
Summary: The Community Resource Navigator will work with a diverse population of individuals requesting resource connection in Marion County and surrounding areas. This position will help program participants and all individuals that seek Marion Matters for assistance in resource navigation through a database system used for reporting. The work of the Community Resource Navigator will be performed under the general supervision of the Program Director.

- Creates and maintains a welcoming environment for clients.
- Responsible for answering the phone, checking voicemail, and managing the common office email.
- Helps clients in accessing tools, training, or services they need to overcome their barrier(s) to stabilization.
- Develops an assistance plan after conducting an oral needs assessment and interview about barriers.
- Works in conjunction with social service agencies, probation departments, and other help-related agencies to help meet needs.
- Identifies additional barriers and assesses appropriateness of training/educational services.
- Follow up with current clients and past graduates to check in on their goal setting and resource navigation.
- Creates and updates manuals and/or computer database client files by recording each contact made with clients and maintains records required for follow-up.
- Keeps current files of community resources and their programs up to date.
Arrange services with providers as required by case notes and plans.
Assist clients in completing written applications for social services when needed.
- Produces written/data reports at monthly staff meetings.
- Participates in Getting Ahead support calls/training.
- Helps the Program Director prepare for Getting Ahead classes, follow up, and graduation.
- All other duties as assigned.
*Position Requirements and Qualifications: *
- Undergraduate degree in Human Services, or Human Resources or related field OR two years related experience and/or training OR equivalent combination of education and experience.
- Work with limited supervision.
- Ability to work well in a team environment.
- Demonstrated ability to track progress and maintain client contact.
- Ability to communicate effectively (oral and written) with staff, employees, management, referral sources, public, etc.
- Ability to write accurate, detailed and concise case notes.
- Knowledgeable in Microsoft WORD, EXCEL, POWERPOINT and CANVA.
- Has reliable transportation
- Ability to lift and carry 25 lbs.
- Ability to work with and be sensitive to people from diverse backgrounds.
Ability to attend and engage in meetings, trainings and professional development opportunities as required.
