SUMMARY OF POSITION:

The Social Worker is responsible to assess patient's social and behavioral needs, provide focused support to patients to address health inequities and aims to address the social, emotional, and environmental factors impacting their well-being. The Social Worker collaborates closely with the clinic physicians, providers and staff, community health workers, and other healthcare professionals to enhance patient outcomes, reduce readmissions, and improve the overall patient experience.

MINIMUM QUALIFICATION(S):

  • Master's degree in Social Work (MSW)

PREFERRED QUALIFICATION(S):

  • Current Social work licensure in Pennsylvania

  • Minimum if two (2) years experience

  • Certification in case management from a nationally recognized organization.
