Private Duty LPN – Greenfield
Location:Greenfield, TN
Schedule:3/4 Split ScheduleShift Hours:6:00 PM – 5:00 AM
Overview:Adoration Home Health is looking for a compassionate and skilled LPN to join our private duty team in Greenfield, TN. This overnight position focuses on one-on-one care, with specific needs for G-tube management and seizure monitoring in a supportive home environment.
Key Responsibilities:
Provide specialized nursing care with G-tube management and seizure monitoring.
Record and track patient vital signs, intake, and output through the night.
Administer medications as prescribed, providing patient and family education as needed.
Collaborate with the healthcare team to ensure a high level of continuity and quality in patient care.

Qualifications:
Active LPN license in Tennessee.
Experience with G-tube and seizure care is preferred but not required.
CPR certification required.

Responsibilities
Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
