POSITION: Behavioral Health Case Manager (BHCM)

EMPLOYMENT STATUS: Full-Time (FT)

POSITION SUMMARY:

Bridgeway Center, Inc. (BCI) is seeking applicants that want to work with clients to effectively address clients' care by assessing needs, developing, monitoring, and facilitating interdisciplinary approaches. This position is responsible for managing an assigned caseload of clients having mental illness diagnosis and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.

The Behavioral Health Case Manager (BHCM) will provide team case management to adults with serious and persistent mental illness to assist the recipient to remain in the community and avoid institutional care. BHCM, also known as Intensive team case managers, coordinate needs assessment, services planning, and provide service oversight.

The Case Management department provides services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in accessing services, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. Case Management includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized, integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. The Case Management department educates persons served and their support system regarding diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.


Applicants must be outgoing and enjoy working with the public, professional, and para-professional team. Have the ability to collaborate and communicate with BCI departments and external stakeholders. Must be able to multi-task and always remain professional to ensure that each client has a positive and helpful therapeutic experience.

MINIMUM REQUIREMENTS:

Bachelor's degree from an accredited university or college with a major in Counseling, Social Work, Psychology, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
