Available Physical Therapist Shifts:
Full-Time : 25+ visits per week
Part-Time : 15-20 visits per week
Per Diem : 5+ visits per week (unlimited, no set goal)
Coverage Area: Openings throughout the state of DE

Key Responsibilities of a Physical Therapist:
As a Physical Therapist , you will:
Perform home visits for clients within your assigned geographic area
Conduct diagnostic tests and mobility assessments (e.g., joint range, strength, balance, stability, ambulation patterns)
Develop and implement personalized care plans —using therapeutic exercises, gait training, and other interventions
Collaborate with a multidisciplinary team for holistic patient care
Educate and empower patients and their families on rehabilitation techniques and safety
Accurately document patient progress and care plans via a state-of-the-art touch pad tablet
What We’re Looking for in a Physical Therapist:
Current Delaware PT license
At least 1 year of recent PT experience in a clinical or home healthcare setting
Graduation from an ACPE-accredited physical therapy program
Ability to work independently and manage time effectively
Excellent interpersonal and problem-solving skills
Solid computer skills ; prior experience with electronic medical records (EMR) is a plus
