MIRACLES OF A SECOND CHANCE Counselor or Counselor/BHT is responsible for providing behavioral health services under the direct supervision of the Administrator/Designee or Clinical Director. The Counselor or Counselor/BHT will be responsible with performance of the following duties:
- Interview the patient and family members with the assistance of the case coordinator to collect social and psychological histories, formulate a social service needs assessment, and plan, and to gather information within seventy-two (72) hours if admitted to RTC.
- Ensure safe, secure and confidential care of each patient on his/her case load.
- Provides individual, group, family and/or Martial therapy under the direction of the attending Physician.
- Demonstrates an understanding of psychiatric terminology.
- Formulate, along with the assistance of the clinical team members of the treatment plan.
- Initiate discharge planning upon admission.
- Chart all entries in the SOAPIE format.
- Outline patients attendance in all aspects of the program.
- Incorporate the Mission Statemen and Values while delivering all aspects of care.
- Maintain a positive verbal relationship with all clinical team members.
- Adhere to and supports all established policies and procedures.
- Demonstrates a professional attitude and exhibits emotional stability.
- Provides urgent care when necessary.
- Provide community consultation.
- Serves as a resource on Standing or Ad Hoc Committees.
- Provides input at Quality Assurance meetings as directed.
- Assist in Staff Development.
- Maintain and improve skills by participation in education seminars/workshops.
