The MIRACLES OF A SECOND CHANCE behavioral health technician/case manager will provide behavioral health services to residents of MIRACLES OF A SECOND CHANCE in accordance with treatment plans, assessments, counseling treatment, and other behavioral health services in accordance with MIRACLES OF A SECOND CHANCE scope of work, program description, and policies and procedures. Supervision will be provided by a State Board of Arizona licensed behavioral health professional or a board certified professional in accordance with R9-10-703.

Duties:

  1. Understand and implement individual treatment plans;
  2. Complete initial assessments;
  3. Record resident behavior;
  4. Provide a safe, supportive environment for residents;
  5. Assist behavioral health professionals with the treatment of residents who have behavioral health problems;
  6. Meet with residents regularly to monitor their well-being, address their questions or concerns, and connect them with resources;
  7. Perform crisis stabilization functions;
  8. Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with recipient, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the individualized service plan (ISP) are implemented;
  9. Brief telephone or face-to-face interactions with a resident, a residents family, or other involved party for the purpose of maintaining or enhancing resident functionality;
  10. Assistance in finding necessary resources other than covered services to meet basic needs;
  11. Serves as a point of contact and to ensure ongoing collaboration including the communication of appropriate clinical information with other involved parties as appropriate and coordination of care with a residents family, behavioral and general medical and dental health care providers, community resources, and other involved supports including educational, social, judicial, community and other State agencies;
  12. Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly;
  13. Participates in staffing, case conferences or other meetings with or without the resident or residents family participating;
  14. Coordinates referral or completes the screening and assessment on all residents on caseload;
  15. Ensures the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health services;
  16. Performs all case management functions associated with caseload including participating in the assessment and service planning processes; including identifying the need for further or specialty evaluations;
  17. Collaborates with the resident and residents family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option;
  18. Maintains the residents comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning);
  19. Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the recipient and input from the resident and relevant others resulting in modification to the service plan as necessary;
  20. Provides transportation to resident as a propriate;
  21. Ensures confidentiality of verbal and written resident information, in accordance with HIPAA standards and network policy;
  22. Adheres to the legal/ethical/professional guidelines adopted by MIRACLES OF A SECOND CHANCE; and
  23. Other duties as assigned.
