Intermountain Health HIM Health Record Analyst in Baton Rouge, Louisiana

Job Description:

Responsible for health record completion activities including, but not limited to, the analysis of the electronic health record, document accuracy and completeness of quality audits, provider notification and suspension communication to Medical Staff Office (MSO), suspension status communications regarding chart completion and providing education and assistance to providers and clinicians in document completion activities. This position ensures the quality of the medical record by verifying completeness, timeliness and accuracy in accordance with policies and procedures. Provides provider and clinical support to ensure documentation is in compliance with medical staff bylaws, compliance requirements, Joint Commission standards, and other government rules and regulations.

Scope

  1. Analyzes the electronic health record for completeness of documentation and accurately prepares computerized deficiencies for inpatient and outpatient records. This is done in compliance with departmental policies and procedures, medical staff bylaws, rules and state and federal regulations. Ensures all actions taken in carrying out responsibilities support patient centered care.

  2. Performs pertinent daily electronic work queue duties of a varying but often complex and time-sensitive nature. This involves working various work queues as assigned. This individual will analyze all verification work queues, as well as monitor all other work queues associated with chart deficiency and perform other duties in regards to chart tracking.

  3. Serves as a liaison to providers/clinicians and advises them on documentation completion requirements per departmental policies and procedures, medical staff bylaws, compliance requirements, Joint Commission standards, rules and regulations. Assists them as necessary to expedite incomplete document processing. This assistance includes having complete knowledge of electronic health record systems.

  4. Monitors and maintains an effective system of identifying/notifying staff of incomplete medical records. Generates and monitors incomplete/delinquent/suspension lists when needed and updates deficiencies in eSummit. Reports provider record completion statistics and collaborates with MSO and manager as needed.

  5. Monitor providers declined workqueue and other workqueues including monitoring the completion of operative reports. This also involves working various workqueues as assigned. Reviews medical records edited by providers and updates deficiencies as needed.

  6. Produces and distributes delinquency notification/suspension letters electronically as needed based on current delinquent record status and current workflows. Verifies accuracy of deficiency information prior to sending letters. Assists providers having issues using the electronic medical record systems with InBasket management issues involving deficiencies.

  7. Regulatory Auditing: Documentation audits performed on a scheduled basis in approved documentation tools.

  8. Facilitates documentation improvement through the identification of documentation issues impacting quality and regulatory compliance requirements.

  9. Promotes mission, vision, and values of SCL Health, and abides by

Education

Required:

High School Diploma or Equivalent required

Preferred:

Associate or Bachelor's Degree in healthcare related field preferred

A degree may substitute for years of experience required

Certification

Preferred:

Registered Health Information Technologist (RHIT) or Registered Health Information Administrator (RHIA) preferred

Experience

Required:

One year previous office experience and/or in a healthcare setting

Preferred:

Experience in a HIM department setting preferred. Health Record Analysis experience strongly preferred

A degree may substitute for years of experience required

  5. Ability to navigate through EMR / Content Management systems (preferably Epic and OnBase) and locate information or identify source of required information.

  6. Ability to work within a team.

  7. Knowledge of Google Workspace.

  8. Detail oriented with critical thinking skills.

  9. Must be able to demonstrate proven problem solving, analytical thinking and customer service. 10. Verbal and written communication skills.

  10. Ability to perform job function and make decisions without direct supervision.

  11. Skilled in personal organization and time management.

  12. Demonstrates ability to learn/use various hospital and clinic information systems at a high level.

  13. Ability to work with all hospital personnel both positively and productively.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
