Johns Hopkins University Community Program Coordinator in Baltimore, Maryland

We are seeking a Community Program Coordinator (Intern) to bring great commitment, creativity, tenacity, and humility to their work within the Hopkins Community Connection (HCC).

The HCC collaborates with communities to challenge systemic barriers to health equity by training a diverse workforce of individuals who champion a more just system through an innovative and patient-centered approach to addressing social determinants of health in the healthcare setting.

Specific Duties & Responsibilities

Work with a caseload of clients professionally and effectively

  • Timely and enthusiastic participation in 15-20 hours of shifts per week at the East Baltimore Medical Center HCC location.

  • Routinely screen patients and families for social needs, navigate to appropriate resources, address barriers as needed, and close the loop with the medical team through accurate and timely EMR documentation.

  • Have equitable and engaging conversations with families that surface information needed to meet their resource goals.

  • Document all work being done with families to provide the greatest possible continuity of care.

  • Effectively manage a caseload of 15-20 patients at all times.

  • Serve as a point of contact and support for HCC advocate volunteers.

Connect families to services located in their community

  • Understand and follow HCC scope of services.

  • Tenaciously use the tools available to you to find the best possible service(s) for the family you are working with.

  • Identify, investigate, and assign said services in concert with families, and create effective plans for those connections.

  • Demonstrate professional and values aligned communication when following up with families on the plans made to connect them to resources at least once every 7 to 10 days until the case has been closed.

Use experience to reflect and grow as a healthcare leader

  • Actively participate in training sessions.

  • Engage in feedback with peer and staff leaders to improve personal, team, and organization performance.

  • Develop skills in project management, communication, and cultural awareness through training and dialogue with peer and staff leaders.

  • Acquire skills in data, people, and systems management through training and use of HCC and JHM tools.

Be an ambassador of Hopkins Community Connection with several different constituencies

  • Understand the work HCC does and communicate it in an accessible way to our client population.

  • Have healthy communication with your clinic community, i.e. physicians, nurses, administrative staff, and behavioral health professionals.

  • Be a representative for HCC in the clinic community by being knowledgeable about and living HCC’s values in their work.

Special Knowledge, Skills, & Abilities

  • Proficiency with the Microsoft Office suite and comfort in technology-rich environments.

  • Experience utilizing Electronic Medical Record systems.

  • Excellent problem-solving skills.

  • Knowledge of landscape of public benefits and community resources and how to facilitate individuals’ access to those resources.

  • Demonstrated ability to engage and work closely with a wide range of individuals, including clinical providers, staff, and patients and families.

  • Superb interpersonal and communication skills (verbal and written).

  • Exceptional initiative and attention to detail.

  • Comfort receiving data-driven performance feedback.

  • Demonstrated ability to excel in a fast-paced, dynamic, diverse, and entrepreneurial environment, lead projects, meet concurrent deadlines, organize time and priorities, and to do so in collaboration with diverse stakeholders.

Minimum Qualifications

  • Bachelor’s Degree required.

  • Two years progressively responsible program or case management experience.

  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

  • Fluency in Spanish preferred.

Starting Salary Range: $22.50 - $40.00 HRLY ($42,000 targeted; Commensurate with experience)
