UPMC Family Hospice is hiring a full-time Home Care Aide. You will work Monday through Friday on daylight shifts from 8:00 a.m. to 4:30 p.m. (Weekends on a rotating schedule).

The home health aide's role is to provide patient-centered care to persons with life-limiting illnesses. The HHA will aid with bathing, dressing, and other activities of daily living. The HHA will also educate and demonstrate care to caregivers to support the goal of remaining at home for the end of life. Quality care is patient-centered, and the HHA goal is to ensure all patients are treated with respect, dignity, and compassion.

There is some scheduling flexibility. One-on-one care and relationship building with peers and patients. Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home. You will have two (2) holiday rotations and weekend rotations every 6-8 weeks.

Responsibilities:

  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff.

  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.

  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.

  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.

  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.

  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

  • Proven ability to facilitate change.

  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.

  • Participates in all required agency meetings.

  • Must maintain annual competency evaluation scores as determined by regulatory requirements

  • High School Diploma or GED - Required

  • One (1) year experience - Preferred

  • Certified Nurse Aide- Preferred

  • Minimum of 75 hours of Nurse Aide training- Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire. Licensure, Certifications, and Clearances: Preferred: Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. Required: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
