Job Summary: We are seeking a highly skilled and compassionate Part Time RN Case Manager to join our team at BrightStar Care of Indianapolis. As a Part Time RN Case Manager, you will be responsible for providing quality and comprehensive care to our clients in the comfort of their own homes. You will work closely with our team to ensure that our clients receive the highest level of care and support. Compensation & Benefits: As a Part Time RN Case Manager at BrightStar Care of Indianapolis, you will receive a competitive hourly pay.  Responsibilities: - Conduct initial assessments for new clients and develop a comprehensive care plan - Provide skilled nursing care, including medication management and wound care, according to physician orders - Coordinate and collaborate with other healthcare professionals, such as therapists and social workers, to ensure the best possible care for our clients - Monitor and evaluate client progress and adjust care plans as needed - Educate clients and their families on proper care and self-management techniques - Maintain accurate and timely documentation of all care provided - Communicate effectively with the BrightStar Care team to ensure client needs are met - Adhere to all policies, procedures, and standards of nursing practice Requirements: -Current and unrestricted RN license in the state of Indiana - Minimum of 1 year of nursing experience, preferably in home health or hospice care - Strong clinical assessment and critical thinking skills - Personal Care Service Experience -Knowledge of Indiana Medicaid a plus.   - Ability to work independently and manage time effectively - Excellent communication and interpersonal skills - Proficient computer skills for documentation purposes - Valid driver's license, reliable transportation, and proof of car insurance   EEOC Statement: BrightStar Care of Indianapolis is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristics protected by law.
